Replacement Certificate-Print/Post reissue

Certificates can only be issued printed and produced in your name as it appears in the student register when you obtained your Unique Student Identifier Number.

The name appearing on a replacement certificate shall normally be the same name which appeared on the original certificate. In cases where the student’s name has changed after the course, contact us to provide documentary evidence or we may be able to verify through www.usi.gov.au. Verification of any change of name is required before a replacement certificate can be issued in a new name.
